If you want the Court to know who you want to be your guardian if that becomes necessary, you may submit a Request to Nominate Guardian Form to the Nevada Lockbox, maintained by the Secretary of State’s office.

Guardianship is used when there are no alternatives for ensuring the care of a child or an incapacitated adult. For adults, one possible way to avoid guardianship is to sign legal documents designating a trusted person to handle their financial or medical affairs while they still can make such decisions.
